Staff-management. In the institutional long-term care setting, staff management techniques have been used with success to improve the continence status of elderly residents. Staff management in the institutional setting involves several components (Schnelle, 1991). , 1990). In phase 1, research assistants were taught to administer prompted-voiding treatment. The staff management system included self-monitoring and recording of promptedvoiding activities, supervisory monitoring by the head nurse, and feedback based on group performance and activities.
Contingency Management. Contingency management, also referred to as behavioral modification, is a consistent and systematic approach of rewarding continent voiding through verbal praise and discouraging incontinence with verbal disapproval (Burgio & Engel, 1987). Contingency management is sometimes combined with the scheduled voiding intervention. Several studies have used prompted-voiding therapy, a form of contingency management with scheduled voiding. This intervention can even be used with cognitively impaired adults.
O'Donnell, Beck, and Walls (1990) reported another noninvasive assessment technique of incontinence. An electronic incontinence sensor was placed in an absorbent pad worn by the patient and the sensor transmitter was positioned for the convenience of the patient. The frequency of incontinence and volume of urine loss were measured continuously for 10 days. Significantly more incontinent episodes occurred during the evening shift when compared to the day and night shifts; however, the largest fluid volume losses occurred on the night shift.
